To request a copy of your health care records from Rush Creek Counseling:

1. Complete a Release of Information form found in your client portal; or
2. Contact the office to complete a printed copy of the Release of Information form.
3. See form titled “Release of Information Fees & Court Policies & Fees” in your client portal for associated fees for release of records.
